DOT Physicals
We are making DOT medical examination requirement inexpensive for the entire fleet
Every commercial driver needs to undergo a physical medical examination in order to renew, or apply for, his/her license. We understand that this is a crucial time and we are here to help you.

What to bring during your exam?
- Driver’s License
- Drink plenty of water as you will need to provide urine sample for urine exam
- Current medications list
- Eyeglasses or contacts for distance or reading (if you are using at home)
- Hearing Aids (if you are using at home)
Additional Documents (in special cases, if applicable)
- Diabetes – either a letter from your treating provider for documentation of HgA1C in the last 2-weeks or the test can be performed at our clinic; For insulin-dependent you will need an ITDM form from the last 45 days (See Below)
- Any Heart Problem – cardiology clearance letter and INR (International Normalized Ratio number) – call us for a referral
- Sleep apnea – CPAP machine usage data of last 30 days
